Kindergarten Registration

One of the following

• Original or Certified Copy of Birth Certificate
• Passport
• Baptismal Record

One of the following

• Driver's License
• State ID
• Work Photo ID
• Passport

A. One (1) of the following with your name and address:

• Current signed lease (rent receipts are not acceptable) or
• Proof of homeownership (settlement statement or mortgage bill)

B. Two (2) of the following with your name and address: 

• Utility bill
• Bank statement
• Insurance bill
• Other business mail (no junk mail)

Please Note: All mail must be dated within 30 days of the registration appointment.

C. Parental Residency & Custody Affidavit 

• The Parental Residency & Custody Affidavit is mandatory and must be notarized.

• Please note, if you do not have a Mortgage or Lease in your name to your current residential address and are residing with a Friend or Family member who is the homeowner, the homeowner must fill out the Owner's Affidavit and have it notarized.

• Renters: The homeowner/landlord must write a letter stating that you and your child are living with the renter on the lease, or you and your child’s name must be added as occupants to the lease.

Records can be brought to school later after doctor appointments

• Copy of your child’s immunization records from their doctor
• Kindergarten Physical Form (Use when registering a new kindergarten student)

Finalized with court stamp (if applicable)

• Divorce Decree
• Guardianship or Custody
• Grandparent Power of Attorney
• Journal Entry – Case Worker (must have district of responsibility listed in the document)

• IEP AND ETR (current copies)
• 504 Plan (current copy)

• Your child must be five years old by August 1, 2025. 

• There is no fee to attend Kindergarten at Fairview Park City Schools (except supply fees) for Fairview Park residents.

• Step 1: Beginning January 22, 2025, please visit our Kindergarten Registration page to begin registration.
• Step 2: Review all the information and then click on the link to Begin Kindergarten Registration.
• Step 3: Fill out the registration in its entirety.
• Step 4: Provide the required documents to the school.

• You will receive school transportation information approximately one week before the start of school. You will learn more about transportation at the parent meeting.

• Prior to the start of school, you will also receive information on how to add funds to your child’s lunch account.

• If families successfully register their student by April 16, 2025,, they will receive a special invitation to our Kindergarten Round-Up event on May 8, when they will have the opportunity to meet some of the Gilles-Sweet Elementary staff. The invitation will be emailed to families.

• The Parkview EEC program offers a limited number of before/after school spots. Please contact the EEC main office at (440) 356-3515 for more information.

• No. Children must attend only half-day kindergarten (2.5 hours per day). Some school districts do offer all-day kindergarten, but if a family requests only half-day kindergarten for a child, the district must accommodate the family’s request.
