A completed form must be provided to the school principal and/or nurse before the student may possess and use an epinephrine autoinjector to treat anaphylaxis in school.
If the student is being treated for diabetes, a completed and signed Diabetes Care Plan must be submitted to the school clinic by your child's healthcare provider using their healthcare system's forms. Questions? Please contact your school's clinic.
