Severe Allergies
If the student has severe/life-threatening allergies or has recently been diagnosed with severe/life-threatening allergies, an allergy action plan must be completed and signed by the doctor and parent/guardian. Forms and medications must be turned in to the school nurse/aid.
A completed form must be provided to the school principal and/or nurse before the student may possess and use an epinephrine autoinjector to treat anaphylaxis in school.
